Location: Home visits will be completed in Santa Rosa, Okaloosa, Walton Counties. May be required to go to doctors' appointments, therapy, etc. very occasionally will go into office in Pensacola, FL (4-5) times per year

Shift: Monday - Friday 8-5 pm EST

Duration: temp to y\hire

LCSW or LMHC is required.

Walk me through the day-to-day responsibilities of this the role and a description of the project:

• Develops, assesses, and facilitates complex care management activities for primarily physical needs members to provide high quality, cost-effective healthcare outcomes including personalized care plans and education for members and their families.
• This person will be placing calls to members / providers.
• Home visits depending on member needs.
• Assessments / charting
• Phone calls and computer-based work when they are not meeting with members. Describe the performance expectations/metrics for this individual and their team:
  • Metric based work.
  • Required to make and complete certain call metrics with the members.
  • Each member must have a completed call or at least (3) attempts (monthly / quarterly)
  • Charting standards Job Description: Position Purpose:
    Supervises the care management team and the care coordination of behavioral health members to promote quality and efficacy of care management delivery related to mental and behavioral health needs. Supervises day-to-day escalations and care management issues related to members or providers.
